The image is 1400x1400, so it's not a size issue, I don't think. We host on Squarespace and have successfully posted our podcast before, but after duplicating our site for updates, we ran into some things that didn't populate over. We've managed to fix most of them, but this one is lingering and prevents our new podcast episodes from being posted to iTunes. Spotify has no problem, but iTunes still doesn't accept it.
